I obtained my PADI open water certificate

I obtained my PADI open water certificate over 9 years ago but i cannot find my card in order to be able to do a refresher course. Is their a database where a record of my passing the course would be kept? I did the course in Malta. I'd appreciate your advice.

There is a C-Card Replacement form that you can download and all the required procedures and contact information to replace your C-Card is on this page.
